This week has real momentum.

The strongest thread is AI getting serious scrutiny in women's health: we've got a new embryo-selection trial for older mothers, but also a sharp opinion piece calling out that AI has optimized the wrong problem in IVF (embryo labs instead of patient dropout), plus a multi-framework analysis revealing which AI tools in fertility are actually ready to deploy versus hype. Meanwhile, two completed trials on postpartum transitions and perinatal mental health prevention show what structured care can do, and new trials are testing everything from vagus nerve stimulation for period pain to community health worker support for pregnancy hypertension. I'd run a post on what the evidence says AI should actually be solving in fertility, then follow with a second on the non-AI wins in postpartum and perinatal care that don't need the technology. No source failures this week.

I found a strong week across AI diagnostics and digital health in women's reproductive medicine. The standout is the scoping review on AI for reproductive health (116 studies, 2021-2025): it's the most comprehensive map we have of this space, and its honesty about bias and validation gaps is exactly what clinicians need to hear before adopting these tools. The plasma proteomics study on adnexal masses (ADAPT-MS) is elegant work, though it's still preprint. Two other preprints worth flagging, the fetal ultrasound framework and the ECG-hormone signatures study, show promise but need peer review before we cite them to readers. On the digital health side, the RCT of the IPT-informed perinatal app is candid: high uptake but no efficacy edge over usual care at 8 weeks, which is the kind of honest result that matters. The menstrual tracking app study raises a real signal about cyberchondria that deserves attention. Three preprints (the antenatal care qualitative work, the French clinician survey, and the tobacco cessation protocol) are early-stage but worth watching.

The Alignment Paradox of Medical Large Language Models in Infertility Care: Decoupling Algorithmic Improvement From Clinical Decision-Making Quality

A study of 8,201 fertility clinic records evaluated whether alignment strategies (supervised fine-tuning, direct preference optimization, group relative policy optimization, and in-context learning) for medical large language models translate from algorithmic correctness to clinical reasoning quality in infertility diagnosis and ART treatment planning.

No new trial results or status changes this week, but I ran 117 trials across the sources and everything is pulling cleanly. Worth noting: when nothing moves, it usually means we're in the waiting period between data lock and publication, or trials are still enrolling. I'll keep watching for the lag to break.

Had a solid week of signals, though some feel half-baked. The NIH grant for that thermal therapy device in Lewisville caught my eye because NIH money means real clinical validation is coming, even if they won't name the company yet. MidlifeCare Africa launched twice in my feed, which tells me someone's pushing the menopause narrative in emerging markets, and that's worth watching. On the regulatory side, Maxigen Biotech got FDA clearance for three water-based lubricants, which is fine but feels like the kind of commodity play that floods the market without moving the needle. Oura's IPO announcement matters more for what it signals about wearables becoming legitimate health infrastructure rather than wellness theater. Several items came in as headlines only, so I'm missing the actual story on what that Women's Health Plan does or what the consumer research actually found.

Money and fundingSun, Sep 27 · 6 kept of 106 new

This week I found a lot of talk about women's health capital but very little hard data to work with. Oura's planned IPO targets 2.2 billion dollars, and there's mention of record grants from the Women's Legacy Fund, but the snippets don't give me the actual figures or where that money is going. The World Economic Forum and Burjeel roundtable both frame women's health investment as growing and necessary, yet neither source provides the funding numbers, allocation breakdowns, or which conditions are seeing the money flow. I'll need fuller articles to tell you where capital is actually landing in women's health research and care.
